Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy care to patients in their homes
Conduct start of care assessments and resumption of care visits
Complete recertifications and daily therapy visits to support patient recovery
Collaborate with clinical coordinators and directors for guidance in the field
Perform occasional safety evaluations or teaching visits in hospice situations
Maintain accurate documentation and follow OASIS guidelines

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home Health experience and OASIS training preferred
First time travelers welcome

Knowledge & Skills

Strong clinical skills in home health physical therapy
Comfortable working independently in patient homes
Excellent communication and documentation abilities
Ability to manage a caseload of 5-6 visits per day
Familiarity with OASIS documentation preferred
