Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Complete approximately 5 meaningful patient visits each day to support recovery goals
Document patient progress using HCHB electronic medical records on your provided tablet
Collaborate with the intake RN and manager to coordinate comprehensive care plans
Help geriatric patients with multiple conditions like CHF and COPD regain independence
Support joint replacement patients through their rehabilitation journey at home

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
HCHB experience preferred
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home Health physical therapy expertise
Orthopedic and geriatric patient care experience
Strong documentation and EMR skills
Excellent communication and patient education abilities
Independent decision-making and time management
