Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide expert physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for orthopedic, joint replacement, and geriatric patients with multiple comorbidities
Complete approximately 5 meaningful patient visits each day using your clinical expertise
Document care accurately using HomeCare HomeBase EMR on your provided tablet
Collaborate with the intake RN and office manager to coordinate exceptional patient care

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong clinical skills in orthopedic and geriatric care

Knowledge & Skills

Home Health physical therapy expertise
Proficiency with HomeCare HomeBase EMR system
Ability to manage a 5-visit daily caseload efficiently
Excellent communication and documentation skills
Comfortable with approximately 40 miles of daily travel within the service area
