Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions
- Help geriatric patients with multiple comorbidities like CHF and COPD regain mobility and independence
- Document patient progress using HomeCare HomeBase EMR on your provided tablet
- Collaborate with the intake RN and office manager to coordinate optimal patient care
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Comfortable working independently in home settings
- Valid driver's license and reliable transportation
Knowledge & Skills
- HomeCare HomeBase (HCHB) EMR system experience
- Home Health PT specialty skills
- Ability to manage a caseload of approximately 5 visits per day
- Strong communication and patient education abilities
- Comfort navigating rural service areas with approximately 40 miles of daily travel
