Candidate Pay Rate: $59 per hour

Roles & Responsibilities
- Provide high-quality home health physical therapy services to patients in their homes
- Develop and implement individualized treatment plans to improve mobility and function
- Collaborate with interdisciplinary care team to ensure coordinated patient care
- Educate patients and families on home exercise programs and safety strategies
- Document patient progress and maintain accurate clinical records
- Travel up to 25% to patient homes within the Bakersfield area
Qualifications
- Active California Physical Therapy license required
- Current BLS certification from the American Heart Association
- Experience in home health or outpatient physical therapy preferred
- Valid driver's license and reliable transportation for local travel
Knowledge & Skills
- Strong clinical assessment and treatment planning skills
- Excellent communication and patient education abilities
- Ability to work independently and manage a caseload
- Knowledge of home health regulations and documentation standards
- Compassionate and patient-centered approach to care
