Position Overview

We are seeking a compassionate and experienced PACE Admissions Registered Nurse (RN) to join the PACE Admissions team. Under the direct supervision of the PACE Admissions RN Supervisor, the RN will support the enrollment and assessment process for a frail elderly population.

This role involves completing Level of Care (LOC) determinations, initial in-home nursing assessments, care coordination, and transition-of-care activities. The position requires travel to participants' homes for required home-site visits.

Key Responsibilities

• Conduct initial In-Home Assessments (IHA) for prospective PACE participants as part of the IHA team.

• Assess the home-care needs of frail elderly participants using the nursing process.

• Develop initial nursing plans of care based on comprehensive assessments.

• Complete and submit Level of Care (LOC) determination forms for supervisor review.

• Review medical records and other relevant clinical information to support LOC determinations.

• Maintain an accurate and up-to-date tracking list of submitted LOCs, including decisions and required follow-up.

• Follow up on deferred prospective enrollees and submit additional information to DHCS as needed.

• Coordinate and implement home-care services when required.

• Coordinate Durable Medical Equipment (DME) for program participants.

• Assist with care transitions and monitor participants during hospitalizations and SNF stays.

• Support appropriate discharge planning and transitions back to the community.

• Communicate effectively with participants, caregivers, and family members.

• Participate in weekly enrollment team meetings, Interdisciplinary Team (IDT) meetings, and other meetings as required.

• Maintain compliance with departmental policies, procedures, regulations, and applicable healthcare standards.

• Participate in educational programs, in-services, workshops, and professional development activities.

• Perform other duties as assigned.

Qualifications

• Current California Registered Nurse (RN) license.

• At least 1 year of RN experience in home health, community health, or a related setting.

• Experience working with frail elderly populations preferred.

• Strong clinical assessment, documentation, communication, and care coordination skills.

• Ability and willingness to travel to participants' homes for required in-home assessments.

• Bilingual skills are not required.

Required Certification

• Current BLS for Healthcare Providers / CPR certification through the American Heart Association, or CPR/AED for the Professional Rescuer certification.

Education

• High School Diploma or GED equivalent required.

Work Environment

This is a Monday–Friday, 8:00 AM–5:00 PM temporary position based in San Diego. The role includes required travel for participant home-site visits and involves working closely with patients, families, caregivers, and the interdisciplinary care team.
