Roles & Responsibilities

Provide compassionate, hands-on nursing care to assigned residents and patients
Administer medications and treatments safely and document accurately
Monitor and record vital signs, reporting changes to the RN or provider
Follow individualized care plans and participate in care planning
Collaborate with nursing staff, therapists, and interdisciplinary team members
Support patients and families with education and clear communication
Maintain infection control practices and a safe care environment
Contribute to continuous improvement and quality initiatives

Qualifications

Current LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or obtain upon hire)
Clinical experience in long-term care, rehab, or acute settings preferred
Ability to complete facility onboarding, background check, and drug screen
Strong communication skills and a patient-focused attitude

Education

Graduate of an accredited practical nursing (LPN/LVN) program

Knowledge & Skills

Medication administration and documentation
Skilled in basic assessments and recognizing changes in condition
Familiarity with wound care and infection control practices
Comfortable using electronic medical records and charting accurately
Good time management, prioritization, and teamwork skills
Patient advocacy and family-centered care approach
