Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents with attention to dignity and comfort
• Administer medications and treatments accurately and document all care promptly
• Monitor patient status, recognize changes, and escalate concerns to RN/Provider as appropriate
• Collaborate with the interdisciplinary care team to support individualized care plans
• Maintain clear, timely charting and communicate effectively with patients, families, and staff
• Support safe infection control practices and facility policies at all times

Qualifications

• Active LPN license (Licensed Practical Nurse) — state licensure expected for practice
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Comfortable with medication administration, basic wound care, and routine assessments
• Friendly, patient-centered approach with solid communication and teamwork skills
• Previous experience in long-term care or skilled nursing preferred but open to strong new grads

Education

• Graduation from an accredited practical nursing program or equivalent

Knowledge & Skills

• Strong fundamentals in direct patient care and basic clinical assessments
• Medication administration and documentation accuracy
• Clear verbal and written communication with patients, families, and team members
• Ability to follow care plans and contribute to interdisciplinary collaboration
• Time management skills to handle an 8.5-hour shift efficiently
