Roles & Responsibilities

• Provide compassionate daily care to residents living with Alzheimer's and dementia
• Administer medications and treatments as ordered by the care team
• Monitor and document residents' health status and report changes
• Collaborate with nurses, physicians, and aides to create personalized care plans
• Support residents with activities of daily living while preserving their dignity
• Educate families about care strategies and safety precautions

Qualifications

• Active LPN license in Washington State or a compact state nurse license
• Current BLS/CPR certification
• Experience in long-term care, geriatrics, or memory care is a plus
• Strong assessment and communication skills
• Ability to work a full-time evening rotation, including weekends
• Completed COVID-19 vaccine series or a documented exemption per facility policy

Knowledge & Skills

• Knowledge of medication administration and accurate documentation
• Understanding of Alzheimer's and dementia care best practices
• Comfort using electronic health records (EHR)
• Strong interpersonal and teamwork skills
• Compassionate, patient-centered approach
• Ability to adapt quickly to changing resident needs
