Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents
Administer medications and treatments accurately following physician orders
Perform routine assessments and document changes in patient condition
Collaborate with the care team to develop and implement individualized care plans
Assist with activities of daily living while promoting patient dignity and independence
Communicate clearly with patients, families, and the interdisciplinary team
Maintain accurate and timely documentation in the medical record

Qualifications

Active LPN license in Pennsylvania (must be in good standing)
Current BLS/CPR certification (or ability to obtain upon hire)
Completion of an accredited LPN program
Ability to pass background check and drug screening
Strong communication skills and a team-oriented attitude
Previous experience in long-term care, rehab, or similar settings preferred

Education

Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital signs monitoring
Wound care fundamentals and infection control precautions
Charting in electronic medical records and accurate record-keeping
Time management and prioritization in a busy clinical environment
Ability to work collaboratively with nurses, CNAs, therapists, and physicians
