Roles & Responsibilities

• Provide compassionate, safe patient care during overnight shifts, monitoring status and documenting changes
• Administer medications and treatments accurately and promptly, following facility protocols
• Perform focused assessments and report findings to RN/MD in a timely manner
• Collaborate with the interdisciplinary team to support resident care plans and transitions
• Maintain accurate, timely documentation in the medical record and communicate shift handoffs clearly
• Support infection control and safety practices to protect patients and staff
• Build positive relationships with residents and families to enhance patient experience

Qualifications

• Active LPN (Licensed Practical Nurse) license in New Mexico (required)
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Minimum clinical experience caring for adult/geriatric populations preferred
• Ability to work nights (7:00 PM - 7:30 AM) on Thursday, Friday, Saturday consistently
• Pass background check, drug screen, and provide up-to-date immunizations as required
• Local candidate only (position designated LOCAL ONLY)

Education

• Completion of an accredited LPN/Vocational Nursing program; high school diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Basic patient assessment and monitoring
• Comfort working overnight with independent decision-making and escalation when needed
• Clear verbal and written communication for handoffs and care coordination
• Knowledge of infection control practices and safe patient handling
• Teamwork mindset and adaptability in a skilled nursing/long-term care environment
