Roles & Responsibilities
- Provide compassionate direct patient care including assessments, medications, and daily nursing tasks
- Administer treatments and document clinical observations accurately and timely
- Collaborate with nurses, therapists, and providers to implement individualized care plans
- Educate patients and families about care plans, medications, and follow-up needs
- Monitor patient status, respond to changes, and escalate concerns to appropriate team members
- Maintain compliance with facility policies, infection control, and safety standards
Qualifications
- Current, active LPN license in New Mexico (required)
- Recent clinical experience as an LPN is a plus; long-term care or skilled nursing experience welcomed
- Basic Life Support (BLS/CPR) certification preferred (or willing to obtain)
- Current immunizations and ability to meet facility onboarding requirements
- Strong communication skills and a team-focused attitude
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or certificate)
Knowledge & Skills
- Proficient in medication administration and accurate charting
- Skilled in patient assessments, vital signs, and basic wound care
- Comfortable using electronic health records and clear documentation practices
- Ability to work independently on assigned shifts and prioritize patient needs
- Good interpersonal skills for patient/family education and team collaboration
