Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document in the medical record
• Perform timely patient assessments and communicate changes to the care team
• Support ADLs (activities of daily living) and promote patient comfort and dignity
• Collaborate with RNs, providers, and interdisciplinary staff to coordinate care
• Maintain clear, accurate documentation and follow facility policies and procedures
• Participate in safety and quality initiatives to help improve patient outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license
• Graduate of an accredited Practical Nursing program or equivalent
• Basic Life Support (BLS/CPR) certification (or willingness to obtain promptly)
• Current immunizations and ability to meet facility health requirements
• Comfortable working the listed schedule (Sunday, Monday, Saturday, 7:00 AM - 7:30 PM)
• Strong communication skills and a team-first attitude; recent clinical experience is a plus

Education

• Practical Nursing diploma or equivalent from an accredited program

Knowledge & Skills

• Solid fundamentals in medication administration and basic wound care
• Competence in patient assessment and recognizing changes in condition
• Familiarity with electronic health records and accurate documentation
• Good clinical judgment, time management, and prioritization skills
• Ability to work collaboratively in a fast-paced healthcare environment
