Roles & Responsibilities

• Provide warm, respectful direct care to residents, assisting with activities of daily living
• Monitor and record vital signs and changes in resident condition with attention to detail
• Support mobility and safe transfers, using proper body mechanics and equipment
• Communicate clearly and compassionately with residents, families, and the care team
• Document care accurately in the chart and report any concerns to nursing staff
• Help maintain a clean, safe, and comfortable environment for residents
• Participate in team huddles and follow care plans to support positive outcomes

Qualifications

• Current GNA / Geriatric Nursing Assistant certification (required)
• High school diploma or GED (preferred)
• Basic Life Support (BLS) certification preferred or willing to obtain
• Experience in long-term care or skilled nursing a plus
• Able to pass background checks, TB screening, and required health screenings
• Strong communication skills and a compassionate bedside manner

Education

• High school diploma or GED required; GNA certification required

Knowledge & Skills

• Skilled in assisting with activities of daily living (ADLs)
• Comfortable taking and documenting vital signs accurately
• Knowledge of infection control and safe patient handling
• Basic charting and documentation skills; familiarity with electronic records a plus
• Good observation skills and ability to report clinical changes
• Team-oriented with strong interpersonal and time-management abilities
