Roles & Responsibilities

• Provide compassionate, resident-centered care and assist with activities of daily living
• Take and document vital signs, reporting any changes to the nursing team
• Support safe mobility, transfers and positioning to promote resident comfort and dignity
• Follow infection control practices and maintain a clean, safe environment
• Complete required documentation accurately and in a timely manner
• Collaborate with nurses and interdisciplinary staff to deliver cohesive care
• Participate in required screenings, fit testing and annual health checks to stay assignment-ready

Qualifications

• Active CNA certification in the state of Indiana (or equivalent) with license/certificate verification
• Current BLS (Basic Life Support) certification
• Compliance with required health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, influenza vaccine, Trinity 10 panel drug screen and fit test
• Trinity color vision check completed as required
• Comfortable working in a long-term care / skilled nursing environment and supporting resident-centered care
• Strong communication skills and a team-first attitude

Education

• High school diploma or GED preferred
• Certified Nursing Assistant (CNA) certification required

Knowledge & Skills

• Solid knowledge of basic nursing care and resident ADLs
• Infection prevention and safe handling techniques
• Ability to take and record vital signs accurately
• Good observational skills and ability to report changes in condition
• Teamwork, reliability and a respectful approach to residents and families
• Basic charting and documentation skills
