Roles & Responsibilities

• Provide hands-on supportive services to nursing staff and other care team members as needed
• Assist residents with activities of daily living (ADLs) including bathing, dressing, toileting, and mobility
• Perform safe lifting, turning, positioning and transfers using proper techniques and equipment
• Coordinate dining room service: set-up/clean-up, deliver trays, assist with feeding and document meal intake
• Deliver and document nutritional supplements and report intake concerns to nursing staff
• Weigh residents per facility policy and report weight changes to the nurse and supervisor
• Monitor skin condition and report pressure areas, skin breakdown or tears promptly
• Complete daily flow sheets and document resident progress accurately in the EMR

Qualifications

• Active Michigan nursing assistant license (must be active at time of submission)
• BLS (American Heart Association) certification required
• High school diploma or equivalent
• Minimum 1+ year of long-term care (LTC) experience required
• Travel experience required for traveler roles
• Demonstrated stable work history (stayed at previous position longer than 13 weeks)
• No disciplinary action in effect against certification
• Resume with permanent address, recent skills checklist, 2 supervisory references from last 2 years

Education

• High School diploma or equivalent required

Knowledge & Skills

• Skilled in ADLs, feeding assistance, toileting programs and resident hygiene
• Competent with patient lifting/transfers and use of assistive devices
• Proficient in documentation, flow sheets and charting (EMR: American Data)
• Able to perform basic clinical measures: weight measurement and skin assessment
• Strong communication skills for reporting resident condition changes to nursing staff
• Team-oriented, dependable, and compassionate approach to long-term care
