Roles & Responsibilities

• Conduct comprehensive evaluations of residents' speech, language, swallowing, and cognitive-communication skills
• Develop and implement individualized treatment plans that improve quality of life and functional independence
• Collaborate with nurses, physicians, occupational therapists, and families to ensure coordinated care
• Document resident progress and adjust treatment approaches to meet changing needs
• Educate patients, families, and staff on communication strategies and safe swallowing techniques
• Provide compassionate, evidence-based therapy in a supportive skilled nursing environment

Qualifications

• Active Illinois Speech-Language Pathologist license
• Certificate of Clinical Competence (CCC-SLP) from ASHA
• Minimum 1-3 years of experience in skilled nursing or similar setting
• Experience working with geriatric populations and patients with dysphagia, aphasia, or cognitive impairments
• Strong clinical and interpersonal skills

Knowledge & Skills

• Knowledge of evidence-based assessment and treatment for swallowing, speech, and cognitive disorders
• Ability to work independently and as part of a multidisciplinary care team
• Excellent communication and documentation skills
• Proficiency with electronic medical records and therapy documentation systems
• A compassionate, patient-centered approach to care
