Roles & Responsibilities

• Provide compassionate hands-on care to LIFE program participants, assisting with activities of daily living
• Perform vital signs, basic clinical observations, and report changes in condition promptly
• Support resident mobility, transfers and safe toileting with dignity and respect
• Follow individualized care plans and communicate effectively with the interdisciplinary team
• Maintain accurate, timely documentation and complete required skill checklists
• Adhere to infection prevention protocols, including respirator fit and PPE use as required
• Participate in required screenings and immunization verification to keep the community safe

Qualifications

• Certified Nursing Assistant (CNA) in good standing — required
• BLS (Basic Life Support) certification — required
• Completion of Trinity Skills Checklist and license/certificate verification prior to start
• Successful completion of required screenings: Trinity respirator evaluation/fit test, 10-panel drug screen, and physical/health statement
• Up-to-date immunizations or documentation: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and seasonal influenza vaccine
• Comfort working in per diem pool environment with flexibility and reliability

Education

• High school diploma or equivalent preferred
• Current CNA certification (state-specific) required

Knowledge & Skills

• Strong resident-centered care skills and respect for dignity and independence
• Competence in basic clinical tasks: vital signs, ADLs, transfers and feeding assistance
• Infection control practices and PPE/respirator use
• Clear verbal and written communication to coordinate with nurses and care team
• Ability to follow care plans, complete skills checklists and accurate documentation
• Reliable, punctual, and able to adapt to changing assignment needs

This is a travel/contract position — explore Delaware’s coastline and charming towns when you’re off shift.
