Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing and long term care setting
• Assess resident conditions, develop and update care plans, and document clinical findings accurately
• Administer medications and treatments safely and according to policy
• Monitor and report changes in resident status to the interdisciplinary team
• Support activities of daily living while promoting dignity and independence
• Collaborate with physicians, therapists, and families to coordinate care
• Participate in infection control, safety practices, and quality improvement initiatives

Qualifications

• Active Registered Nurse (RN) license in good standing for the state of practice
• Experience or interest in Skilled Nursing Facility (SNF) and Long Term Care (LTC) settings
• Up to date immunizations and health requirements: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, current physical/health statement
• Successful completion of Trinity 10 Panel Drug Screen and background checks as required
• Current BLS certification; ACLS or other certifications a plus
• Strong communication skills and ability to work independently in a fast paced environment

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
• Active RN license required

Knowledge & Skills

• Thorough knowledge of long term care nursing practices and regulations
• Medication administration, wound care, and basic clinical procedures
• Skilled assessment, critical thinking, and accurate documentation
• Familiarity with electronic medical records and care planning
• Infection prevention and control best practices
• Ability to collaborate effectively with interdisciplinary teams and families
