Roles & Responsibilities

• Provide skilled physical therapy assistance to long-term care residents in a SNF setting
• Assist with patient evaluations, treatment plans, mobility training, and therapeutic exercises
• Support safe transfers, gait training, and use of adaptive equipment to maximize independence
• Document patient progress accurately and timely in the facility EMR
• Collaborate with therapists, nursing, and interdisciplinary team members to coordinate care
• Educate patients and families on home programs and safe mobility strategies
• Maintain equipment, follow infection control protocols, and uphold facility policies

Qualifications

• Active California Physical Therapy Assistant (PTA) license or eligibility to obtain one
• Experience working in a Skilled Nursing Facility or long-term care environment preferred
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfortable completing full-day shifts within the 7 AM to 7 PM timeframe, Monday-Friday
• Strong documentation skills and ability to communicate clearly with patients and team
• Reliable, professional, and able to handle the physical demands of patient care

Education

• Graduate of an accredited Physical Therapy Assistant program (Associate degree or equivalent)

Knowledge & Skills

• Hands-on experience with therapeutic exercise, gait training, and transfers
• Knowledge of long-term care patient needs and SNF workflow
• Proficient with patient documentation and common EMR tasks
• Skilled in safe patient handling techniques and use of assistive devices
• Good clinical judgment, time management, and teamwork abilities
• Strong interpersonal skills for working with residents and families
