Roles & Responsibilities

• Provide high-quality physical therapy services to patients in a skilled nursing facility
• Develop and implement individualized treatment plans to improve mobility and function
• Collaborate with interdisciplinary team to ensure comprehensive patient care
• Document patient progress and adjust treatments as needed
• Educate patients and families on therapeutic exercises and home care
• Maintain a safe and positive environment for recovery

Qualifications

• Active Physical Therapist license in New Hampshire (or compact eligibility)
• Graduate of an accredited Physical Therapy program
• Previous experience in skilled nursing or rehabilitation setting preferred
• Current BLS certification
• Strong communication and interpersonal skills
• Ability to pass a background check and drug screen

Knowledge & Skills

• Expertise in orthopedic and geriatric rehabilitation
• Knowledge of evidence-based practice and treatment modalities
• Proficient in electronic medical records (EMR) documentation
• Compassionate patient-centered care approach
• Excellent problem-solving and critical thinking skills
