Roles & Responsibilities

• Deliver high-quality physical therapy services to patients in a skilled nursing and rehabilitation setting
• Conduct comprehensive evaluations and develop personalized treatment plans
• Implement therapeutic exercises and modalities to improve mobility, strength, and function
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
• Educate patients and their families on home exercise programs and fall prevention
• Maintain timely and accurate documentation in the EMR

Qualifications

• Must hold an active Physical Therapist license in New Hampshire (or have the ability to obtain one before start)
• Graduation from an accredited Physical Therapy program (Doctorate preferred)
• Current BLS/CPR certification
• Previous experience in a skilled nursing, rehab, or outpatient setting is a plus
• Strong clinical reasoning and problem-solving skills

Knowledge & Skills

• In-depth knowledge of physical therapy modalities and evidence-based practices
• Excellent interpersonal and communication skills
• Ability to work independently and collaboratively
• Proficiency with electronic medical records (EMR)
• Compassionate, patient-first approach
