Location: Modesto, CA

Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans that advance recovery and independence
- Provide skilled therapy interventions to improve mobility, strength, balance, and functional outcomes
- Document evaluations, treatments, and patient progress accurately and in a timely way
- Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes
- Ensure compliance with facility policies and state regulations while advocating for patient-centered care
Qualifications
- Active Physical Therapist license in the state of California (required)
- Skilled Nursing Facility experience preferred but not required
- Strong clinical reasoning, communication, and documentation skills
- Patient-focused and outcome-driven approach to care
Knowledge & Skills
- Proficient in patient evaluation and treatment planning
- Patient-centered care and clinical autonomy
- Accurate documentation and familiarity with clinical record-keeping
- Strong collaboration and communication with interdisciplinary teams
- Understanding of SNF workflows and regulatory compliance
