Physical Therapist - Skilled Nursing Facility
Location: Lancaster, CA
Contract Type: Contract

Candidate Pay Rate: $59.5 - $59.5 per hour

Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans focused on recovery
- Provide skilled therapy to improve mobility, strength, balance, and functional independence
- Document patient progress and treatment outcomes accurately and timely
- Collaborate with interdisciplinary healthcare teams to coordinate optimal care
- Educate patients and caregivers on therapy plans and home exercise programs
- Adjust treatment plans based on patient response and clinical judgment
Qualifications
- Active California Physical Therapist license
- Experience in a Skilled Nursing or rehabilitation setting preferred
- Strong clinical skills with a focus on patient-centered care
- Excellent communication skills to interact with patients, families, and team members
- Ability to work independently while contributing effectively as part of a team
Knowledge & Skills
- Patient evaluation and assessment
- Treatment planning and individualized therapy programs
- Mobility, strength, and balance interventions
- Skilled documentation and tracking of treatment outcomes
