Roles & Responsibilities

• Evaluate and treat skilled nursing facility patients to improve mobility, strength, and function
• Develop and implement individualized treatment plans based on patient goals and clinical findings
• Complete timely, facility-standard documentation in Net Health
• Collaborate closely with nursing and the interdisciplinary care team to coordinate patient-centered care
• Manage caseload and prioritize time effectively to meet patient and facility needs
• Float to a sister facility within walking distance when coverage is required

Qualifications

• Active CA PT license required
• SNF experience preferred; mentorship available for clinicians with 0–1 years of experience
• Strong documentation skills (experience with Net Health is a plus)
• Excellent time-management and organizational skills
• Flexible, collaborative mindset and willingness to float as needed
• Ability to work clinically with a diverse SNF patient population
