Roles & Responsibilities

• Provide comprehensive physical therapy services to post-acute patients
• Develop individualized treatment plans based on patient assessments
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Document progress and outcomes in electronic health records
• Educate patients and families on home exercise programs and self-care strategies

Qualifications

• Licensed Physical Therapist in California
• Bachelor's or Master's degree in Physical Therapy
• Clinical experience in post-acute or acute care settings preferred
• Strong communication and teamwork skills

Knowledge & Skills

• Patient assessment and evaluation
• Therapeutic exercise prescription
• Manual therapy techniques
• Use of rehabilitation equipment and technology
• Interdisciplinary collaboration
• Documentation and charting
