Roles & Responsibilities

• Assess patients' mobility, strength, balance and functional needs to create meaningful plans of care
• Develop and implement individualized therapeutic exercise and mobility programs
• Deliver hands-on treatment and use therapeutic modalities to improve outcomes
• Document progress and outcomes clearly and on time, keeping the interdisciplinary team informed
• Collaborate with nurses, physicians and caregivers to coordinate safe discharge plans
• Educate patients and families on home programs, fall prevention and safe transfer techniques
• Monitor responses to treatment and adjust plans to maximize function and independence

Qualifications

• Current California Physical Therapist license or eligibility to obtain state licensure
• Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy
• Basic Life Support (BLS) / CPR certification required (or willingness to obtain quickly)
• Experience in acute rehab, skilled nursing or outpatient orthopedics is a strong plus
• Excellent communication skills and comfort working in an interdisciplinary team
• Ability to manage documentation and caseload in an efficient, organized manner

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy acceptable

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise and manual therapy techniques
• Strong knowledge of mobility training, gait training and assistive device use
• Familiarity with documentation standards and clinical outcome measures
• Ability to instruct patients and caregivers clearly and compassionately
• Good clinical decision-making and time-management skills in a fast-paced setting
