Roles & Responsibilities

• Deliver personalized physical therapy sessions to patients recovering from illness or injury.
• Develop and implement individualized rehabilitation plans that promote functional independence.
• Collaborate closely with physicians, nurses, and occupational therapists to coordinate comprehensive care.
• Track patient progress, adjust treatment protocols, and document outcomes in the electronic health record.
• Educate patients and families on home exercise programs and injury prevention strategies.

Qualifications

• Licensed Physical Therapist with a valid state license.
• Minimum of 0 years of experience in a skilled nursing or rehabilitation setting.
• Strong communication and interpersonal skills.
• Ability to work effectively within an interdisciplinary team.

• Patient assessment and evaluation
• Therapeutic exercise prescription
• Manual therapy techniques
• Modalities such as ultrasound and electrical stimulation
• Documentation and charting
• Interdisciplinary collaboration
