Location: San Rafael, CA
Contract Type: Perm
Candidate Pay Rate: $59.5 - $59.5 per hour

Roles & Responsibilities
- Assess residents' functional abilities and craft individualized care plans that make a real difference
- Deliver skilled occupational therapy services to both short-term and long-term residents
- Document treatment outcomes accurately and clearly to support continuity of care
- Participate actively in care planning meetings as a valued member of the interdisciplinary team
Qualifications
- Active California Occupational Therapist license required
- SNF experience preferred
- Excellent interpersonal skills and strong clinical documentation abilities
- Ability to collaborate effectively with rehab team and facility staff
Knowledge & Skills
- Skilled in occupational therapy assessment and individualized care planning
- Experience providing OT interventions for short- and long-term care residents
- Clear, accurate clinical documentation skills
- Strong communication and teamwork within interdisciplinary care settings
- Familiarity with SNF care environments and resident-focused rehabilitation
