Roles & Responsibilities

• Assess patient function and develop individualized occupational therapy plans that promote independence
• Provide hands-on treatment, adaptive equipment recommendations, and skilled interventions in the SNF setting
• Document progress clearly and timely in patient records while tracking outcomes
• Collaborate with nurses, PTs, SLPs, and care teams to coordinate patient-centered rehab plans
• Educate patients and families on strategies for safe transfer, daily living activities, and home setup
• Monitor caseloads to ensure consistent, high-quality care and identify opportunities for long-term improvement
• Participate in team meetings and contribute to a supportive, growth-focused rehab culture

Qualifications

• Licensed Occupational Therapist (OTR/L) in California or eligible to obtain licensure
• Degree in Occupational Therapy (BS/OT or MS/OT/OTD) from an accredited program
• Experience in skilled nursing facility, geriatric rehab, or post-acute care preferred
• Current CPR/BLS certification (or willingness to obtain)
• Strong clinical reasoning, documentation skills, and ability to manage a consistent caseload
• Excellent communication and teamwork skills — you’ll be working closely with interdisciplinary teams

Education

• Bachelor's, Master's, or Doctorate in Occupational Therapy (required)

Knowledge & Skills

• Skilled in assessment and intervention planning for ADLs, transfers, and functional mobility
• Knowledge of adaptive equipment, splinting basics, and home safety recommendations
• Comfortable with outcome-driven documentation and discharge planning
• Ability to collaborate effectively with PTs, SLPs, nursing, and case management
• Strong patient- and family-education skills with a compassionate approach
• Time management and caseload prioritization in a SNF environment
• Familiarity with common SNF patient diagnoses and rehab progression
