Location: Merced, CA
Contract Type: Perm
Shift Type: Monday-Friday, Day Shift (flexible)
Hours per Week: 40
Candidate Pay Rate: $59.5 - $59.5 per hour

Roles & Responsibilities
- Evaluate residents and design individualized occupational therapy treatment plans
- Deliver therapeutic interventions to improve activities of daily living (ADLs) and functional mobility
- Collaborate with interdisciplinary care teams to coordinate comprehensive resident care
- Maintain accurate documentation in line with state and facility guidelines
- Educate residents and caregivers about treatment plans, progress, and strategies for independence
Qualifications
- Active California Occupational Therapy (OT) license (required)
- SNF experience preferred; new graduates are welcome to apply
- Strong clinical reasoning and clear communication skills for working with residents and care teams
Knowledge & Skills
- Skilled in resident evaluation, assessment, and individualized treatment planning
- Experience improving ADLs, transfers, and functional mobility
- Comfortable working within interdisciplinary teams and care pathways
- Accurate clinical documentation and knowledge of state/facility requirements
- Ability to educate residents and caregivers in an encouraging, practical way
