Occupational Therapist - Skilled Nursing Facility
Location: Madera, CA
Contract Type: Perm
Candidate Pay Rate: $59.5 - $59.5 per hour

Roles & Responsibilities
- Perform thorough evaluations and re-evaluations to assess resident needs
- Design and implement individualized occupational therapy treatment programs
- Ensure compliance with regulatory standards and maintain accurate documentation
- Collaborate closely with physical therapists, speech-language pathologists, nursing staff, and physicians to deliver coordinated care
- Support rehabilitation goals for geriatric residents and promote functional independence
Qualifications
- Active California Occupational Therapy license (required)
- SNF or geriatric experience preferred
- Ability to work independently and as part of a multidisciplinary team
- Strong patient-centered approach and effective communication skills
Knowledge & Skills
- Occupational therapy assessment and treatment planning
- Geriatric patient care and rehabilitation
- Regulatory compliance in skilled nursing settings
- Teamwork and interdisciplinary collaboration
