Roles & Responsibilities

• Provide primary medical management in collaboration with PACE physicians
• Perform participant assessments and develop individualized plans of care
• Evaluate and treat participant physical complaints and follow-up as needed
• Document clinical encounters thoroughly and keep the interdisciplinary team informed
• Deliver caregiver and participant teaching to support independent living
• Oversee daily operations of the PACE clinic and coordinate care across settings
• Conduct rounds in contracted nursing homes and communicate findings to the team
• Participate actively as a member of the Interdisciplinary Team (IDT) to optimize care

Qualifications

• Active Nurse Practitioner credentials (NP certification) — ability to practice in Indiana is expected
• Experience in geriatric care is required
• Comfortable providing phone-based on-call coverage (week-on/week-off rotation)
• Proven ability to work independently, exercise clinical judgment, and adapt to change
• Strong written and verbal communication skills for both in-person and virtual collaboration
• Familiarity with EMR systems — currently TruChart with planned transition to Epic

Education

• Graduate-level preparation as a Nurse Practitioner (MSN/NP or equivalent preferred)

Knowledge & Skills

• Primary medical management and clinical decision making for older adults
• Patient assessment, care plan development, and focused documentation
• Care coordination and interdisciplinary team collaboration
• Experience with EMR workflows (TruChart now; Epic coming soon)
• Telephone triage/on-call communication and documentation
• Comfortable with Microsoft Office tools (Word, Excel, PowerPoint) and virtual platforms
