Roles & Responsibilities

• Provide compassionate personal care to program participants, promoting dignity and independence
• Assist with activities of daily living (bathing, dressing, toileting, grooming) with respect and patience
• Monitor and record vital signs and routine observations accurately
• Support mobility and safe transfers while following established safety protocols
• Communicate clearly with interdisciplinary team members and document care in required checklists
• Participate in Trinity Specialty Specific Competency Testing and skills checklists to maintain quality care
• Adhere to infection control practices and facility policies to protect participants and staff

Qualifications

• Current Certified Nursing Assistant (CNA) certification or equivalent
• BLS (Basic Life Support) certification required
• Ability to pass Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• License/certificate verification on file as required by site
• Up-to-date health screening and immunizations (2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza)
• Clear 10-panel drug screen and required health documentation

Education

• High school diploma or equivalent
• Certified Nursing Assistant (CNA) certificate

Knowledge & Skills

• Comfortable providing hands-on personal care and assisting with ADLs
• Strong communication and documentation skills
• Knowledge of basic infection control and safety procedures
• Ability to take and record vital signs accurately
• Team-oriented mindset and ability to work with interdisciplinary staff
• Compassionate bedside manner and respect for resident rights
