Roles & Responsibilities

• Provide compassionate, direct patient care and assist residents with activities of daily living
• Safely measure and record vital signs and patient observations
• Assist with mobility, transfers, and positioning while following proper body mechanics
• Maintain accurate, timely documentation of care and resident status
• Follow infection control protocols and complete required screenings and testing
• Participate in Trinity Specialty Specific Competency Testing and maintain required certifications
• Communicate clearly with nurses, care team members, residents, and families to support continuity of care
• Contribute to a positive, respectful care environment and support facility quality goals

Qualifications

• Current CNA certification (state-certified) — required
• BLS (Basic Life Support) certification — required
• License/Certificate Verification required prior to start
• Completion of Trinity Specialty Specific Competency Testing as part of onboarding
• Willingness and ability to complete health requirements: fit test, 2-step PPD or IGRA, drug screen, immunizations and related paperwork
• Strong communication, teamwork, and a genuine desire to provide respectful patient-centered care
• Reliable, punctual, and able to work flexible per diem shifts

Education

• Current CNA certification (required)
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of activities of daily living (ADLs) and resident-centered care
• Infection prevention and proper PPE use
• Basic clinical skills: vital signs, patient transfers, feeding and hygiene assistance
• Familiarity with documentation best practices and facility charting systems
• Ability to follow care plans and report changes in resident condition
• Good interpersonal skills and professional bedside manner
• Comfort with competency testing processes and periodic re-evaluation
