Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents following facility policies
- Administer medications and treatments accurately and safely
- Document resident status, care provided, and changes in condition clearly and promptly
- Collaborate with the care team to develop and follow individualized care plans
- Monitor residents for changes and escalate concerns to the RN or provider as needed
Qualifications
- Active LPN license in New York (required)
- Completion of an accredited practical nursing program
- Current BLS/CPR certification or willingness to obtain upon hire
- Ability to pass background checks and meet facility onboarding requirements
- Strong communication skills and a caring, patient-centered approach
Education
- Graduation from an accredited practical nursing program; LPN/LVN certificate or diploma
Knowledge & Skills
- Clinical skills in medication administration and basic nursing procedures
- Familiarity with documentation standards and electronic charting preferred
- Ability to observe and report changes in resident condition
- Teamwork and effective communication with interdisciplinary staff
- Time management and prioritization in a busy care setting
