Roles & Responsibilities
- Provide compassionate daily nursing care to residents and patients
- Administer medications and treatments accurately and on schedule
- Monitor and document vital signs, patient status, and care activities
- Collaborate with RNs, CNAs, and interdisciplinary teams to coordinate care
- Educate patients and families about care plans and health maintenance
- Maintain accurate and timely clinical documentation in the patient chart
- Support admissions, discharges, and safe care transitions
Qualifications
- Current LPN license in Pennsylvania (PA) required
- Basic Life Support (BLS) certification required or obtained upon hire
- Comfortable with medication administration and basic nursing procedures
- Strong communication and teamwork skills
- Professional, compassionate approach to resident and patient care
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation
- Vital sign assessment and basic patient monitoring
- Wound care basics and infection control practices
- Clinical documentation and electronic health record familiarity
- Patient and family education with clear communication
- Ability to work effectively within a multidisciplinary team
