Roles & Responsibilities
- Provide safe, compassionate direct patient care as an LPN
- Administer medications and treatments according to plan and policy
- Monitor and document vital signs, patient status, and care given
- Communicate changes in condition clearly to the care team
- Assist with care planning and support resident comfort and dignity
- Collaborate with nurses, therapists, and support staff to deliver quality care
- Maintain accurate clinical documentation and adhere to facility protocols
Qualifications
- Valid Licensed Practical Nurse (LPN) license for New York (required)
- Current BLS/CPR certification (or willingness to obtain, per facility policy)
- Ability to pass background checks and meet facility onboarding requirements
- Strong interpersonal skills and a patient-centered attitude
Education
- Completion of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and basic wound care
- Vital signs assessment and patient monitoring
- Clear, compassionate patient and family communication
- Clinical documentation and care plan implementation
- Teamwork and the ability to follow established clinical protocols
