Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments safely and accurately
- Monitor and document changes in patient conditions and report concerns promptly
- Collaborate with RNs and the interdisciplinary care team to deliver coordinated care
- Assist with daily living activities, admissions, transfers, and discharges as needed
- Maintain accurate clinical documentation and medication records in the patient chart
- Support a positive, respectful environment that promotes dignity and comfort
Qualifications
- Active Licensed Practical Nurse (LPN) license in New York (required)
- Current BLS/CPR certification (or willingness to obtain)
- Previous LPN experience in long-term care, skilled nursing, or related setting preferred
- Strong communication skills and the ability to work well in a team
- Ability to pass background check and any required health screenings
Education
- Completion of an accredited Practical Nursing / LPN program
- High school diploma or GED (standard prerequisite)
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care and vital signs monitoring
- Familiarity with infection control and patient safety protocols
- Experience with electronic medical records (EMR) is a plus
- Good time management, organization, and patient advocacy skills
