Roles & Responsibilities
- Administer medications safely and accurately using MAR and TAR documentation
- Provide compassionate patient care and assist with activities of daily living
- Chart in PCC and maintain clear, timely patient records
- Monitor patient status, report changes to the care team, and escalate concerns when needed
- Collaborate with nurses, providers, and support staff to deliver coordinated care
- Follow infection control practices and facility safety policies to protect patients and staff
- Support admissions, transfers, and discharges with professional, patient-centered care
Qualifications
- Current Licensed Practical Nurse (LPN) credential (must be credentialed and local to the area)
- Demonstrated experience with medication administration
- Required knowledge of PCC, MAR, and TAR (these are must-haves for this role)
- Comfortable working overnight 12.5-hour shifts
- Up-to-date immunizations, background check, and any facility-required onboarding documentation
Education
- LPN diploma or practical nursing certificate (required)
Knowledge & Skills
- Proficiency with PCC electronic charting
- Accurate MAR and TAR documentation skills
- Strong medication administration and safety practices
- Solid clinical assessment and patient observation skills
- Clear communication and teamwork with interdisciplinary staff
- Time management for overnight patient care and documentation
