Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Monitor resident conditions and communicate changes to the care team promptly
• Support activities of daily living and encourage resident independence
• Participate in competency testing and maintain required certifications
• Maintain a safe, clean environment and follow infection control protocols
• Collaborate with nursing staff and leadership to deliver person-centered care

Qualifications

• Current Licensed Practical Nurse (LPN) license — required
• BLS (Basic Life Support) certification — required
• Completion of Trinity Specialty Specific Competency Testing (per checklist)
• Successful license/certificate verification on file
• Ability to complete required health screenings: fit test, 2-step PPD or IGRA, influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine
• Negative Trinity 10 panel drug screen and completed physical/health statement
• Trinity respirator evaluation and Trinity color vision testing as applicable
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN / Vocational Nursing program

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Knowledgeable about infection prevention and resident safety practices
• Ability to accurately document care in resident charts and electronic records
• Comfortable working in long-term care / assisted living environments
• Good clinical judgment and ability to escalate concerns appropriately
• Teamwork, time management, and interpersonal skills
