Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN scope of practice
• Administer medications and timed treatments accurately and document per facility policy
• Monitor and record vital signs, report changes in condition promptly to the care team
• Support safe admissions, discharges, and transitions with clear documentation and handoffs
• Collaborate with nurses, therapists, and support staff to deliver individualized care plans
• Champion resident safety, infection control practices, and quality improvement initiatives

Qualifications

• Current Licensed Practical Nurse (LPN) credential required
• Active nursing license valid for practice in New Hampshire preferred
• Ability to clear background checks and required immunizations per facility policy
• Excellent communication skills and a patient-centered attitude
• Previous long-term care or skilled nursing experience is a plus

Education

• Graduation from an accredited practical nursing program (required)

Knowledge & Skills

• Medication administration and documentation best practices
• Monitoring and interpreting vital signs and basic clinical assessments
• Wound care fundamentals and applying physician-directed treatments
• Accurate charting and familiarity with electronic health records
• Strong teamwork, time management, and resident-centered communication
• Infection prevention and adherence to facility policies
