Roles & Responsibilities

• Assess patients and families using a holistic approach to identify social, emotional, and environmental needs
• Provide crisis intervention and supportive counseling to patients and their families
• Coordinate discharge planning and community resources to promote safe transitions of care
• Document assessments, care plans, and interventions clearly and promptly in the medical record
• Advocate for patients' needs with clinical teams, community agencies, and families
• Deliver culturally sensitive care, engaging patients and families in meaningful ways
• Supervise or cover supervisory duties as needed to ensure continuity of social services

Qualifications

• MSW (Master of Social Work) from an accredited school of Social Work
• Licensed in Alaska at the Master level (LMSW - Alaska required)
• Minimum one (1) year experience in a hospital or health-care setting
• Valid Alaska state driver’s license
• Basic computer skills for documentation and communication
• Preferred: cross-cultural experience and bilingual English & Yup'ik language skills
• Ability to present information clearly in one-on-one and group settings
• Mathematical competency for basic computations and data interpretation

Education

• MSW (Master of Social Work) from an accredited school of Social Work

Knowledge & Skills

• Medical social services and hospital social work practice
• Case management and care coordination
• Assessment and treatment planning
• Clear, compassionate communication with patients, families, and care teams
• Accurate documentation and clinical record keeping
• Ability to work independently and perform supervisory tasks when needed
• Cultural humility and experience in cross-cultural care
