Roles & Responsibilities

• Provide compassionate, skilled nursing care to home health patients—make their day better and safer
• Perform clinical tasks such as medication administration, wound care, and vital sign monitoring with precision
• Document patient care accurately and efficiently in Cerner so the team stays informed
• Educate patients and families on care plans and self-care techniques to promote independence
• Collaborate with interdisciplinary team members to coordinate excellent post-acute care
• Maintain required certifications (BLS) and adhere to safety and infection control standards
• Participate in orientation and ongoing training to continuously improve clinical practice

Qualifications

• Active LPN/LVN license (eligible to submit license)
• Minimum 2 years of relevant nursing experience (explicit requirement)
• BLS (Basic Life Support) certification required
• Experience with Cerner charting preferred
• Home health or post-acute care experience preferred
• BSN not required (explicitly stated)

Education

• Completion of an accredited LPN/LVN program (diploma/certificate); BSN not required

Knowledge & Skills

• Strong patient assessment and clinical judgment
• Proficient charting/documentation in Cerner
• Skilled in medication administration and wound care
• Knowledge of home health care practices and post-acute needs
• Excellent communication and patient/family education skills
• Ability to work independently while collaborating with the care team
