Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients' homes, focusing on safe mobility and functional independence
• Complete accurate OASIS assessments and timely EPIC and Rover charting for each visit
• Manage a caseload typically up to 6 patient visits per day while maintaining high-quality care
• Coordinate care with interdisciplinary team members and communicate clearly with patients and caregivers
• Educate patients and families on home exercise programs, fall prevention, and safe home modifications
• Maintain required certifications, including BCLS, and adhere to facility vaccination requirements
• Plan routes and manage travel time safely and efficiently, minimizing windshield time when possible

Qualifications

• Current Physical Therapist license for the state of Oregon (pending license accepted)
• Minimum 1 year of skilled home health experience required
• BCLS certification required
• OASIS experience required; please highlight if you do not have OASIS experience
• Comfortable using EPIC remote client and Rover mobile app for documentation
• Reliable transportation for home visits and ability to travel up to approximately 60 miles in a day
• Facility requires primary COVID-19 vaccination series plus one booster

Knowledge & Skills

• Skilled in home health physical therapy practices and safety assessments
• Proficient with OASIS processes and EPIC charting workflows
• Comfortable using mobile apps like Rover for field documentation
• Strong patient education and caregiver coaching skills
• Excellent time management and route planning to manage multiple visits
• Good clinical judgment for discharge planning and care coordination
