Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination for patients in need of mental health support
Conduct thorough assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient transitions
Support patients and families through the 72-hour involuntary evaluation process when appropriate
Maintain accurate documentation and advocate for patient rights and safety

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities strongly preferred
Two supervisory references needed at time of submittal

Knowledge & Skills

Expertise in involuntary psychiatric holds and mental health crisis intervention
Strong assessment and care coordination abilities
Excellent communication and documentation skills
Ability to work collaboratively within a multidisciplinary team
