Roles & Responsibilities

• Assess residents' communication, swallowing, and cognitive-communication needs with compassion and clinical rigor
• Develop and implement individualized treatment plans that improve function and quality of life
• Provide skilled speech-language therapy interventions in a skilled nursing environment
• Document progress accurately and timely to meet facility and regulatory standards
• Collaborate with nursing, therapy colleagues, and families to coordinate care
• Educate residents and caregivers on strategies to carryover gains outside of therapy sessions
• Meet the facility productivity expectation (90%) while delivering quality care
• Adhere to Continuum Rehabilitation Group policies and procedures and facility dress code (black scrubs)

Qualifications

• 1-3 years of experience as an SLP (as listed in the job) — new grads with strong clinical skills may inquire
• Appropriate SLP credentials and state licensure as required by the facility or state
• Comfortable working in a skilled nursing facility (SNF) practice setting
• Willingness to follow CRG policies and productivity expectations
• Ability to be present in the building to receive guaranteed hours

Knowledge & Skills

• Skilled in SLP assessment and treatment planning for adults in SNF settings
• Strong documentation and time management skills
• Excellent communication and teamwork abilities
• Knowledge of dysphagia management, cognitive-communication therapy, and restorative approaches
• Ability to meet productivity targets while maintaining patient-centered care
