Roles & Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement discharge plans to ensure smooth transitions
• Provide crisis intervention and support for patients in psychiatric emergencies
• Collaborate with interdisciplinary teams to coordinate care
• Facilitate 5150 evaluations and ensure compliance with California regulations
• Advocate for patients' needs and connect them with community resources

Qualifications

• Active LCSW license in California
• Current BLS certification
• Valid 5150 certification in California (with proof of passing exam with 80%)
• Minimum 2 years of experience in social work, preferably in acute care
• Prior experience with Dignity Health (CA) required
• Experience with Cerner charting system

Knowledge & Skills

• Deep understanding of 5150 involuntary holds and psychiatric evaluation processes
• Strong crisis intervention and de-escalation skills
• Knowledge of discharge planning and community resources
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a fast-paced hospital environment
• Familiarity with electronic health records (Cerner)
