Roles & Responsibilities

• Evaluate patients' physical therapy needs and develop individualized treatment plans
• Implement therapeutic exercises and modalities to improve mobility, strength, and function
• Educate patients and their families on treatment plans, home exercises, and injury prevention
• Document patient progress accurately and adjust treatments as needed
• Collaborate with the healthcare team to ensure comprehensive patient care
• Maintain a safe and supportive environment for patients and staff

Qualifications

• Current and valid Physical Therapist license in the state of New Hampshire
• Graduation from an accredited physical therapy program
• CPR/BLS certification (preferred)
• Previous experience in a skilled nursing or rehab setting is a plus
• Must be able to pass a background check and bring valid identification

Knowledge & Skills

• Knowledge of therapeutic exercises, manual therapy, and modality techniques
• Strong patient assessment and care planning skills
• Excellent communication and interpersonal abilities
• Ability to work effectively as part of a multidisciplinary team
• Proficient in documentation and electronic health records
