Roles & Responsibilities

Deliver compassionate home health care to 5-6 patients daily, making a real difference in their lives
Conduct skilled assessments and treatments in patients' homes across Franklin and Macon County
Collaborate with interdisciplinary teams to create personalized care plans
Document care meticulously using HCHB system for seamless continuity
Educate patients and families to empower independence and better health outcomes

Qualifications

Active RN license in good standing
2+ years of nursing experience
Reliable vehicle for patient home visits
Home health and hospice experience preferred

Education

Associate or Bachelor's degree in Nursing (ADN/BSN)
Current RN licensure

Knowledge & Skills

Strong home health nursing skills with a patient-centered approach
Proficiency in HCHB documentation system
Excellent assessment and care planning abilities
Comfortable working independently in home settings
